Highlights from #AASTAnnualMeeting - Pressure, Perfusion and Compartments

Dr. Rao Ivatury gave a Master Surgeon Lecture on the various manifestations of Compartment Syndrome-

Intracranial Hypertension is a form of Compartment Syndrome

Orbital Compartment Syndrome may result after ocular burns and massive fluid resuscitation.  It can be treated with a lateral canthotomy.

Mediastinal Compartment Syndrome occurs after prolonged cardiac surgery.

Intraabdominal Hypertension / Abdominal compartment Syndrome is well known to most trauma surgeons.

Now there is Poly-compartment syndrome - when ICP, IAP and CVP all rise as a result of our fluid therapy to support the Cerebral Perfusion Pressure.

In extremity compartment syndrome, the classic P's may occur late.  The more P's' that are present, the more likely there is compartment syndrome, but the best diagnosis is measurement of compartment pressure.

There is a syndrome of secondary compartment syndrome - this can occur in the abdomen or in the extremities after massive fluid resuscitation with no injury to that compartment.
